Teeth whitening is one of the few grooming categories where the active chemistry is well understood, the effective products are cheap, and most of the money spent is wasted on formats that cannot work.
The variable that determines results is peroxide concentration multiplied by contact time. Almost everything else — LED lights, charcoal, activated formulas, whitening pens — is either supporting the delivery of that equation or substituting marketing for it.
Extrinsic vs Intrinsic Staining
There are two different problems, and they need different tools.
Extrinsic stains sit on the outside of the enamel, bound into the pellicle — the thin protein film that forms on teeth continuously. These come from chromogens in coffee, tea, red wine and tobacco, and they respond to mechanical removal: brushing, a hygienist's polish, or a mildly abrasive whitening toothpaste.
Intrinsic stains sit inside the tooth, in the dentin under the enamel. These come from aging (enamel thins with time and the yellower dentin beneath shows through), from trauma, from fluorosis, and from tetracycline exposure during tooth development. No amount of brushing touches them, because the discoloration is not on the surface.
This is the single most useful thing to understand before buying anything: whitening toothpaste works on extrinsic stains only. If your teeth have yellowed with age, a whitening toothpaste will not fix it, and peroxide is the only over-the-counter route that will.
How Peroxide Actually Whitens
Hydrogen peroxide is a small molecule, small enough to diffuse through enamel into the dentin beneath. Once there, it oxidizes the large, conjugated organic molecules responsible for discoloration — breaking their double bonds and splitting them into smaller fragments that do not absorb visible light in the same way. The stain is not lifted out; it is chemically dismantled in place.
Carbamide peroxide, used in most tray systems, is a delivery vehicle rather than a different active. It decomposes into hydrogen peroxide and urea at a ratio of roughly one to three — so a 10% carbamide peroxide gel delivers somewhere near 3.5% hydrogen peroxide, released more slowly. That slower release is why trays are worn for longer than strips.
The practical consequence: when comparing products, convert everything to hydrogen peroxide equivalent and multiply by wear time. A 10% carbamide tray worn for an hour and a 3.5% hydrogen peroxide strip worn for an hour are doing approximately the same job.
Why Sensitivity Happens — And What To Do
Peroxide does not stop at the dentin. It continues through the dentinal tubules — the microscopic channels running from the enamel junction toward the pulp — and can reach the pulp itself, producing a transient inflammatory response. That is the sharp, cold-triggered zing that shows up two or three days into a whitening course.
It is usually reversible and usually manageable:
- Potassium nitrate works by raising the potassium concentration around the nerve, which interferes with repolarization and effectively blunts the signal. This is why professional-grade gels frequently include it and why a potassium nitrate toothpaste used alongside a whitening course helps materially.
- Fluoride supports remineralization of enamel during and after treatment.
- Reduce frequency before you reduce the course. Switching from daily to every other day usually resolves sensitivity while still completing the course, just over a longer period.
What Whitening Will Not Do
Peroxide has no effect on crowns, veneers, composite bonding or porcelain. They are not organic tooth structure and there is nothing in them for peroxide to oxidize. If you have restorations in the visible smile zone, whitening the surrounding natural teeth will make those restorations stand out as comparatively darker, and the only fix is replacing them to match. This is a genuine reason to speak to a dentist before starting rather than after.
Gray discoloration — particularly tetracycline banding — responds considerably worse than yellow discoloration. Yellow-brown stains oxidize readily; gray ones often do not shift meaningfully with over-the-counter concentrations.

It will not produce the shade change a strip course produces — contact time is a couple of minutes rather than half an hour — but as a maintenance product holding a result you have already achieved, it is doing real chemistry rather than just polishing. PAP (phthalimidoperoxycaproic acid) is a peroxide-free oxidizing agent marketed specifically on not producing the pulpal sensitivity that peroxide does. The honest position: the mechanism is plausible and the sensitivity advantage is widely reported by users, but the published evidence base behind PAP is considerably thinner than the decades of data behind hydrogen peroxide, and independent comparisons of whitening efficacy are limited. If peroxide has made whitening intolerable for you, this is a reasonable thing to try. If it has not, the better-evidenced option is the strip. Best for: men who cannot tolerate peroxide sensitivity.

Not a whitening product, and included because the most common reason men abandon a whitening course halfway is sensitivity rather than dissatisfaction. Running a potassium nitrate toothpaste for a week before starting, and throughout the course, is the cheapest intervention available for finishing what you started. Best for: anyone starting a peroxide course, known sensitive teeth.
Protocol and Maintenance
Clean first. Peroxide reaching a film of plaque is peroxide not reaching enamel. Brush and floss before applying, but not aggressively immediately before — abraded gums plus peroxide is an unpleasant combination.
Mind the first 48 hours. Whitening temporarily increases enamel permeability, which means teeth are more receptive to staining right after a session, not less. Coffee, red wine and tobacco in that window will undo work you have just paid for.
Do not stack abrasives. Running a high-abrasion whitening toothpaste daily alongside a peroxide course adds mechanical wear on enamel that is already chemically active. Use one or the other.
Expect rebound. Shade regresses somewhat in the weeks after any course as the teeth rehydrate and new staining begins. A well-executed course plus maintenance holds far better than repeated aggressive courses.
