Sonicare and Oral-B dominate electric toothbrushes, and both clean far better than a manual brush. The real difference is how they move: Oral-B's small round head oscillates and rotates around each tooth, while Sonicare's oval head sweeps side to side at very high speed. That difference affects how they feel, how you use them, and who each one suits.
Below we compare the technology, what the clinical evidence actually says, and the models that matter from each brand, from flagship to budget. Prices come from Amazon at our last check.
Choose Oral-B if you want the brushing motion with the strongest clinical evidence, a round head that cups each tooth, and detailed app coaching. The iO line is quieter and smoother than older Oral-B brushes.
Choose Sonicare if you prefer a gentler, quieter sensation, have sensitive gums, braces, implants or bridges, or want the longest battery life. Ask your dentist if you have specific dental work.
Best of each: Oral-B iO9 and Sonicare 9900 Prestige. Best value: Oral-B iO2 and Sonicare 4100.

Oscillating-Rotating vs Sonic: How They Clean
Oral-B (oscillating-rotating). A small round head turns back and forth and pulses as it does, scrubbing at the point where bristles meet the tooth. You hold it on each tooth in turn and let the head do the work. Oral-B's iO models replace the older gear-driven motor with a magnetic drive, which Oral-B says is much smoother and quieter.
Sonicare (sonic). An oval, manual-shaped head sweeps side to side at very high speed. Philips quotes 62,000 brush movements a minute for the 9900 Prestige. The speed also moves fluid between teeth, which Philips calls Fluid Action. You glide it along the teeth more like a manual brush.
What the evidence says. A 2014 Cochrane review of 56 trials (Yaacob et al.) concluded that powered toothbrushes reduce plaque and gingivitis more than manual brushes in both the short and long term, and that the largest body of evidence, with significant reductions at both time points, was for rotation-oscillation brushes. The authors noted the clinical importance of the differences is unclear. In plain terms: both brands beat a manual brush, and Oral-B's technology has the most research behind it.

Which Should You Buy?
- Maximum plaque removal: Oral-B iO.
- Sensitive gums or receding gumline: Sonicare, or an iO on its Sensitive mode; both brands' pressure sensors protect the gums.
- Braces, implants, crowns or bridges: ask your dentist; many prefer the gentler sonic action.
- Travel: Sonicare for battery life; Oral-B iO7 and iO9 for a charging travel case.
- Coaching: Oral-B's app tracking and light ring are the most detailed.
Brush Heads and Running Costs
Plan on a new head about every three months: Oral-B's listings say so, and it is the standard dental advice. Heads are not interchangeable between the brands, or between Oral-B's iO and older Oral-B models, so factor replacement heads into the price of whichever system you choose.
How We Compared
We compared each brand's published specifications, the clinical evidence (cited above) and our electric toothbrush guide. We did not test toothbrushes hands-on for this article.
FAQ
Is Sonicare or Oral-B better?
Both clean far better than a manual brush. Oral-B's oscillating-rotating design has the largest body of clinical evidence; Sonicare's sonic motion is gentler and usually lasts longer per charge. Choose by feel, gums and dental work.
Do dentists recommend Sonicare or Oral-B?
Both are widely recommended. If you have braces, implants or gum problems, ask your dentist which motion suits your mouth.
Can I use Sonicare heads on an Oral-B?
No. Heads are brand-specific, and Oral-B iO heads only fit iO handles.
Is an expensive electric toothbrush worth it?
The cleaning motion is the same across each brand's range; flagships add sensors, coaching, modes and accessories. A mid-range model with a pressure sensor and timer is enough for most people.
